Every week, a new patient sits across from us and asks the same question: "How much weight will I actually lose?" It is the right question. Here is what the science actually says, what we see in clinical practice, and what factors determine where you land on the spectrum.
What the Clinical Trials Show
The landmark clinical trials for both medications produced results that genuinely surprised the medical community — results larger than anything previously seen in non-surgical weight loss.
Semaglutide (STEP-1 Trial): Participants lost an average of 14.9% of body weight over 68 weeks at the full 2.4mg dose. For a 200-pound person, that is approximately 30 pounds. For a 250-pound person, closer to 37.
Tirzepatide (SURMOUNT-1 Trial): At the highest 15mg dose, participants lost an average of 22.5% of body weight over 72 weeks. For a 200-pound person, that is 45 pounds. For some participants, losses exceeded 25% — territory previously only achievable with bariatric surgery.
These are averages. Some participants lost significantly more. Some lost less. The range is wide — and understanding what determines your individual result is more useful than fixating on the average.
What Actually Determines Your Results
Your starting hormonal environment. Patients with significant insulin resistance, hypothyroidism, or hormonal imbalances tend to respond differently than metabolically healthy individuals. Addressing these underlying conditions alongside GLP-1 therapy consistently improves outcomes.
Which medication you use. Tirzepatide's dual GLP-1 and GIP mechanism produces larger average losses than Semaglutide alone — though individual response varies and Semaglutide remains highly effective for many patients.
Dose. Both medications are titrated upward gradually. Patients who tolerate the higher maintenance doses tend to see greater results. Dose escalation strategy matters.
Diet quality. GLP-1 medications dramatically reduce appetite, but they do not eliminate the impact of food quality. Patients who use reduced appetite as an opportunity to improve nutrition — more protein, fewer processed carbohydrates — consistently outperform those who simply eat less of the same foods.
Resistance training. Building and preserving lean muscle mass during weight loss protects your metabolic rate and improves long-term outcomes significantly.
Setting Realistic, Meaningful Expectations
For most of our patients, a realistic expectation for the first 12 months is 10 to 20% of starting body weight — with Tirzepatide-treated patients more likely to land at the higher end of that range. That is meaningful, life-changing weight loss for the majority of people.
But the number on the scale is only part of the story. The metabolic improvements that accompany this level of weight loss — reduced insulin resistance, improved blood pressure, lower triglycerides, better sleep, more energy, reduced inflammation — can be as clinically significant as the weight itself.
1. Start with realistic expectations and a clear timeline. Most patients see meaningful results within 8 to 12 weeks and reach their peak results between 9 and 18 months.
2. Use the reduced appetite strategically. Increase protein intake. Improve food quality. The medication gives you the window — how you use it determines your long-term success.
3. Add resistance training. Protecting and building muscle during weight loss is one of the most important things you can do for long-term metabolic health.
4. Address underlying hormonal issues. If thyroid, estrogen, or insulin issues are present, treating these alongside GLP-1 therapy improves results meaningfully.
