It is one of the most common questions we hear from patients who are considering stopping their GLP-1 medication — or who have already stopped and are watching the scale move in the wrong direction. What actually happens when you stop Ozempic or Tirzepatide? The honest answer is nuanced, clinically important, and far more manageable than most online sources suggest.
What the Research Shows

The STEP-4 trial — a landmark study specifically designed to examine what happens after Semaglutide withdrawal — found that participants who stopped the medication after 20 weeks regained approximately two-thirds of their lost weight within one year. Metabolic markers including blood pressure, blood sugar, and cholesterol also returned toward baseline.
Similar findings have been observed with Tirzepatide. The biology is straightforward: GLP-1 medications work by amplifying a hormonal signal. When the medication is discontinued, that amplified signal goes away. If the underlying metabolic and hormonal conditions that made weight loss difficult in the first place have not been addressed, the body tends to return toward its previous state.

1. Do not stop without a transition plan. Work with your provider to taper rather than stop abruptly, and establish a maintenance protocol before discontinuing.
2. Use the treatment period to build habits. The appetite reduction window is the ideal time to establish higher-protein eating patterns, consistent resistance training, and sleep hygiene — all of which support maintenance.
3. Address underlying hormonal issues before stopping. If insulin resistance, thyroid dysfunction, or hormonal decline contributed to the original weight gain, these need to be treated and stabilised independently.
4. Consider maintenance dosing. Some patients transition to a lower maintenance dose rather than stopping entirely — a clinically supported strategy for preserving results.
5. Have realistic expectations. Some weight regain after discontinuation is normal and does not mean the treatment failed. Maintaining even 50 to 60% of lost weight represents a significant and meaningful health improvement.
